1.) Download and print the Subscriber Site Agreement. Download here 2.) Enter all appropriate information in the spaces provided on the agreement with signature witnessed by a Notary Public. 3.) Include the initial setup fee and advanced monthly payment with the completed agreement (see agreement for payment and fee amounts). If you prefer, an escrow account may be established for automatic payment upon written request submitted on Exhibit A of the Agreement. 4.)
